About this medicine

What is Palnat 100 mg Capsules?

Palbociclib is a targeted medicine for breast cancer that is hormone-receptor-positive and HER2-negative and has spread or come back. It is always taken together with a hormone therapy — an aromatase inhibitor such as letrozole as first treatment, or fulvestrant after hormone therapy has stopped working — and it makes that hormone therapy work for longer.

It blocks two enzymes (CDK4 and CDK6) that cancer cells need to move through the cell-division cycle. It comes as capsules and as tablets: the capsules must be taken with food, the tablets can be taken with or without; otherwise they are the same medicine at the same doses.

Palbociclib is taken once a day for 21 days, then 7 days off, in 28-day cycles. Low white blood cells are the main side effect and are checked before each cycle and mid-way through the first two. It is a prescription-only medicine; a valid prescription must be provided at checkout.

How it works

How does Palbociclib work?

Cells decide whether to divide at a checkpoint controlled by two enzymes, CDK4 and CDK6, together with a protein called cyclin D. In hormone-receptor-positive breast cancer, oestrogen signalling pushes that checkpoint open. Palbociclib blocks CDK4 and CDK6, so the cell is held before the checkpoint and cannot copy its DNA or divide. Hormone therapy reduces the oestrogen signal at the same time, which is why the two are used together — the cancer is blocked at two points of the same pathway. Healthy dividing cells, especially in the bone marrow, are held up too, so white cell counts fall, but they recover during the week off.

CDK4/6 blockade

Palbociclib occupies the ATP pocket of CDK4 and CDK6, preventing them from phosphorylating the retinoblastoma protein (Rb).

Checkpoint held

Un-phosphorylated Rb keeps E2F transcription factors switched off, so the cell cannot pass from G1 into S phase.

Growth arrest

Tumour cells stop dividing; combined with endocrine therapy that removes the oestrogen drive, the arrest is deeper and lasts longer.

Uses

What is Palbociclib used for?

HR-positive, HER2-negative advanced or metastatic breast cancer

With an aromatase inhibitor as initial endocrine therapy, or with fulvestrant after progression on endocrine therapy. In pre- or perimenopausal women, with an LHRH agonist as well. Also in men.

Dosage

How to take Palnat 100 mg Capsules

Your oncologist will prescribe your exact dose. The right dose for you depends on your weight, kidney and liver function, and — for some medicines — your genetic test results.

Always take exactly as prescribed. Do not change your dose or stop taking this medicine without speaking to your doctor first, even if you feel well.

Missed a dose? Skip a missed or vomited dose and take the next one at the usual time. Do not double up.

With food? Capsules: take with food. Tablets: with or without food. Swallow whole with water; do not open, crush or chew. Avoid grapefruit and grapefruit juice.

Important safety information

Safety & warnings

Please read this section before taking Palnat 100 mg Capsules and discuss any concerns with your oncologist.

Common side effects

Side effects affect some people, not everyone. Most are manageable — let your treatment team know about anything that concerns you.

  • Low white blood cells (neutropenia)
  • Infections (usually mild — colds, urinary infections)
  • Tiredness (fatigue)
  • Nausea, reduced appetite
  • Mouth soreness (stomatitis)
  • Anaemia, low platelets
  • Hair thinning
  • Diarrhoea
  • Rash, dry skin

Contact your doctor immediately if you notice:

  • Fever of 38°C or higher, chills, or signs of infection Go to the emergency department — neutropenic infection is uncommon with palbociclib but is a medical emergency when it occurs.
  • New or worsening breathlessness, dry cough, chest pain Seek urgent review — lung inflammation (interstitial lung disease / pneumonitis) is rare but serious; palbociclib is stopped if confirmed.
  • Unusual bruising or bleeding Contact your team the same day — possible low platelets.
  • Mouth ulcers that stop you eating or drinking Contact your team; supportive treatment and a dose change may be needed.

Important: tell your doctor about these medicines

Taking Palbociclib with certain other medicines can be dangerous. Always tell every doctor, dentist, and pharmacist about all medicines you are taking.

  • Strong CYP3A inhibitors (ketoconazole, itraconazole, clarithromycin, ritonavir) and grapefruit Major

    Increase palbociclib levels.

  • Strong CYP3A inducers (rifampicin, phenytoin, carbamazepine, St John's wort) Major

Reduce palbociclib levels and may make it less effective.

Pregnancy & breastfeeding

Palbociclib can harm an unborn baby. It should not be used in pregnancy, and women who could become pregnant should use effective contraception during treatment and for at least three weeks after the last dose.

Do not breastfeed during treatment or for three weeks after the last dose.

Palbociclib may reduce fertility in men. Discuss sperm banking before starting if this matters to you.

FAQ

Frequently asked questions

Is Palbociclib the same as Ibrance?

Palbociclib is the active ingredient in Ibrance. A generic Palbociclib medicine contains the same active ingredient at the same strength and is taken the same way — the main difference is cost. Always use it as prescribed by a licensed medical professional.

Is Palnat 100 mg Capsules chemotherapy?

No. It is a targeted therapy that pauses the cell-division cycle. It does lower white blood cell counts — the reason for the week off and the blood tests — but severe infections are uncommon, and sickness and complete hair loss are not typical.

[2] NCI Drug Information: Palbociclib

Why three weeks on, one week off?

Palbociclib holds dividing bone-marrow cells as well as cancer cells, so white cell counts fall during the three weeks on. The week off lets them recover before the next cycle. Blood counts are checked before each cycle and on day 15 of the first two cycles; if they are too low the next cycle waits a week.

Capsules or tablets — does it matter?

Same medicine, same doses. The capsules must be taken with food (and not with stomach-acid reducers on an empty stomach); the tablets can be taken with or without food. Do not swap between them without telling your pharmacist.

Do I keep taking letrozole or fulvestrant during the week off?

Yes. Only palbociclib pauses; the hormone therapy continues every day (or on its injection schedule). Stopping it defeats the point of the combination.

[4] Macmillan Cancer Support: Palbociclib (Ibrance)

How does Palnat 100 mg Capsules compare with ribociclib and abemaciclib?

All three are CDK4/6 inhibitors used with hormone therapy. They differ mainly in side-effect profile — palbociclib and ribociclib lower white cells more; abemaciclib causes more diarrhoea and is taken continuously; ribociclib needs ECG monitoring. Your oncologist chooses based on your health and what is available.

[3] EMA: Ibrance — European public assessment report

Is Palnat 100 mg Capsules the same as Ibrance?

Palnat 100 mg Capsules contains the same active ingredient, palbociclib, at the same strength as the originator brand Ibrance. Generic medicines must meet the same regulatory standards for quality, safety and effectiveness. Used as prescribed by a licensed medical professional.

[3] EMA: Ibrance — European public assessment report

Can men take Palnat 100 mg Capsules?

Yes — hormone-receptor-positive breast cancer in men is treated the same way, with palbociclib plus an aromatase inhibitor or fulvestrant, often with an LHRH agonist.

Do I need a prescription to order Palnat 100 mg Capsules?

Yes. Palbociclib is prescription-only. Upload your oncologist's prescription at checkout; our pharmacist team reviews it before dispatch. Used as prescribed by a licensed medical professional.
