The most common and clinically important side effect of palbociclib is a reduction in white-blood-cell counts (neutropenia), which increases infection risk and is monitored with regular blood tests. Other reported effects include fatigue, nausea, mouth sores (stomatitis), hair thinning, reduced appetite and low red-blood-cell or platelet counts. Serious effects are less common but can occur. Palbociclib is one of three CDK4/6 inhibitors used in HR-positive, HER2-negative breast cancer, alongside abemaciclib — the active ingredient in Verzenio — and ribociclib.
