Glecaprevir/pibrentasvir is a pan-genotypic direct-acting antiviral combination used in the treatment of chronic hepatitis C. It is sold by its originator as Mavyret in the United States and as Maviret in the United Kingdom, Europe and Australia. Generic versions containing the same two active ingredients at the same strengths are produced by licensed pharmaceutical manufacturers and are prescription-only, as the brand is.

Each tablet contains glecaprevir 100 mg and pibrentasvir 40 mg. The two act at different points in the hepatitis C virus lifecycle: glecaprevir inhibits the NS3/4A protease the virus needs to process its own proteins, and pibrentasvir inhibits NS5A, a protein involved in viral replication and assembly. Combining the two is what makes the regimen pan-genotypic — it is used across hepatitis C genotypes 1 to 6 rather than being matched to a single genotype.
How glecaprevir/pibrentasvir works
The regimen is taken as three tablets once daily, with food — a detail worth noting, because several other hepatitis C combinations are a single tablet daily and the two are not interchangeable in how they are taken. Treatment is given for a defined course rather than indefinitely: eight weeks is common for people who have not been treated before and do not have cirrhosis, with longer courses used in other circumstances. The course length, and whether this regimen is suitable at all, is decided by a specialist after genotype and liver assessment.
Who it's prescribed for
This is a prescription-only medicine for adults and adolescents diagnosed with chronic hepatitis C, prescribed and monitored by a hepatologist or other specialist. It is one of the options that can be considered in people with severe renal impairment, where some other direct-acting antiviral regimens are more limited — but that is a clinical judgement, not a self-assessment. Why the course length matters more than the price per tablet

Hepatitis C treatment is priced by the course, not by the month, so the number of weeks moves the total more than the price of any single pack does. An eight-week course is materially less medicine than a twelve-week one, which is part of why glecaprevir/pibrentasvir was priced differently from the sofosbuvir-based regimens that came before it.
