What is Dolutegravir?
Dolutegravir (DTG) is an integrase strand transfer inhibitor (INSTI) and one of the preferred first-line agents for HIV-1 treatment per WHO and US DHHS guidelines. It offers a high barrier to resistance, once-daily dosing, and is well tolerated. It is available in fixed-dose combinations including Triumeq (DTG/ABC/3TC) and Dovato (DTG/3TC).

Dolutegravir binds directly to the integrase active site, chelating the two metal ions required for catalytic activity.

By blocking the integrase active site, DTG prevents the viral DNA from integrating into the host cell's chromosomal DNA — a step essential for productive HIV infection.

High resistance barrier
DTG has a high barrier to resistance because it dissociates slowly from integrase. Resistance mutations that emerge under other INSTIs do not confer high-level resistance to DTG.

How to Take
Take with or without food. Avoid taking within 2 hours of antacids, calcium, or iron supplements.

With Food
Food increases exposure slightly but is not clinically significant. High-calcium food/supplements — take DTG 2 hours before or 6 hours after.
Alcohol
No specific interaction, but alcohol can affect adherence

Take as soon as remembered. If within 20 hours of next dose, skip the missed dose. Never double dose.
