About this medicine

What is Jakavi 20 mg Tablets?

Ruxolitinib is a targeted tablet for a group of blood conditions called myeloproliferative neoplasms — mainly myelofibrosis, where scar tissue builds up in the bone marrow and the spleen enlarges, and polycythaemia vera, where too many red cells are made, when hydroxyurea has not worked or is not tolerated. It is also used for acute and chronic graft-versus-host disease after a stem-cell transplant when steroids have not been enough.

It blocks two enzymes, JAK1 and JAK2, that carry the over-active growth signal in these conditions. For most people the first things to improve are spleen size, night sweats, itching, bone pain and tiredness.

Ruxolitinib is taken twice a day with or without food, and the dose is set — and adjusted — from your platelet count and blood counts. It must not be stopped suddenly. It is a prescription-only medicine; a valid prescription must be provided at checkout.

How it works

How does Ruxolitinib work?

Blood-cell production is switched on by signals that pass through enzymes called Janus kinases (JAK1 and JAK2). In myelofibrosis and polycythaemia vera a mutation — most often JAK2 V617F — leaves that signal stuck on, so the marrow overproduces cells, the spleen enlarges to cope, and inflammatory chemicals cause sweats, itching and weight loss. Ruxolitinib blocks JAK1 and JAK2 whether or not the mutation is present, turning the signal down. It does not remove the mutation or cure the condition, but it controls the symptoms and spleen size for most people. Because the same enzymes are needed for normal blood-cell production, counts can fall and the dose is balanced against them.

JAK blockade

Ruxolitinib competes with ATP at the JAK1 and JAK2 kinase domains, preventing them from phosphorylating STAT proteins.

Signal turned down

With STAT signalling reduced, the drive to overproduce blood cells and inflammatory cytokines falls, regardless of JAK2 mutation status.

Clinical effect

Spleen size and constitutional symptoms improve, usually within weeks; normal haematopoiesis is dampened too, so counts are monitored.

Dosage

How to take Jakavi 20 mg Tablets

Your oncologist will prescribe your exact dose. The right dose for you depends on your weight, kidney and liver function, and — for some medicines — your genetic test results.

Always take exactly as prescribed. Do not change your dose or stop taking this medicine without speaking to your doctor first, even if you feel well.

Missed a dose? Skip a missed dose and take the next one at the usual time. Do not double up.

With food? Take with or without food, with water. Swallow whole. Avoid grapefruit.

Important safety information

Safety & warnings

Please read this section before taking Jakavi 20 mg Tablets and discuss any concerns with your oncologist.

Common side effects

Side effects affect some people, not everyone. Most are manageable — let your treatment team know about anything that concerns you.

  • Anaemia (low red cells)
  • Low platelets
  • Bruising
  • Dizziness, headache
  • Raised cholesterol and triglycerides
  • Weight gain
  • Urinary tract infections
  • Raised liver enzymes
  • Shingles (herpes zoster)
  • Wind, constipation

Contact your doctor immediately if you notice:

  • Fever, chills, cough, or a painful blistering rash on one side of the body Contact your team the same day — serious infections, including shingles, tuberculosis reactivation and hepatitis B reactivation, can occur.
  • Severe tiredness, breathlessness, pale skin Contact your team — anaemia may need a dose change or transfusion.
  • Unusual bruising or bleeding, tiny red spots on the skin Contact your team the same day — possible very low platelets.
  • Sudden return of sweats, bone pain, spleen pain or fever after stopping Contact your team urgently — a withdrawal flare; ruxolitinib should be tapered, not stopped abruptly.
  • Chest pain, sudden breathlessness, calf pain or swelling Seek emergency care — blood clots and major cardiovascular events have been reported with JAK inhibitors.
  • New confusion, weakness, vision or speech problems Seek emergency care — progressive multifocal leukoencephalopathy has been reported rarely.
  • New skin lumps or changes in a mole Report at your next visit — other skin cancers are seen more often; use sun protection.

Important: tell your doctor about these medicines

Taking Ruxolitinib with certain other medicines can be dangerous. Always tell every doctor, dentist, and pharmacist about all medicines you are taking.

  • Strong CYP3A4 inhibitors (ketoconazole, itraconazole, clarithromycin, ritonavir) and grapefruit Major

    Increase ruxolitinib levels.

  • Fluconazole (a dual CYP3A4/CYP2C9 inhibitor) Major

    Increases ruxolitinib levels markedly.

  • Strong CYP3A4 inducers (rifampicin, phenytoin, carbamazepine, St John's wort) Major

Reduce ruxolitinib levels and may make it less effective.

  • Other immunosuppressants and biologics Major

    Additive infection risk.

  • Live vaccines Major

    Risk of infection; response reduced.

Pregnancy & breastfeeding

Ruxolitinib should not be used in pregnancy. Women who could become pregnant should use effective contraception during treatment and for at least one week after the last dose.

Do not breastfeed during treatment or for two weeks after the last dose.

Effects on human fertility are not established.

Storage

How to store Jakavi 20 mg Tablets

Temperature

Store below 30°C (86°F).

Light & moisture

Keep in the original packaging.

Keep away from children

Keep out of the sight and reach of children.

Safe handling

Anticancer medicine: swallow whole, do not crush; wash hands after handling. Return unused tablets to a pharmacy.

Travelling

Carry in original packaging with your prescription and a haematologist's letter. Take enough for the whole trip plus a margin — running out means an abrupt stop, which can cause a symptom flare.

FAQ

Frequently asked questions

Is Jakavi 20 mg Tablets chemotherapy?

No. It is a targeted therapy that turns down an over-active signalling enzyme. It does not cause hair loss or the sickness typical of chemotherapy, though it can lower blood counts.

[2] NCI Drug Information: Ruxolitinib Phosphate

Why is my starting dose based on my platelet count?

Ruxolitinib dampens normal blood-cell production as well as the abnormal drive, and platelets are the count most likely to fall. Starting lower when platelets are already low, then adjusting to response, keeps the balance. Counts are checked every two to four weeks until stable.

Why must I never stop Jakavi 20 mg Tablets suddenly?

When ruxolitinib stops, the symptoms it was controlling — sweats, bone pain, enlarged spleen — can return within days, sometimes more severely than before, with fever and inflammation. If it has to be stopped (for surgery, for example) your haematologist will usually taper the dose. Never run out.

[4] Macmillan Cancer Support: Ruxolitinib (Jakavi)

Does Jakavi 20 mg Tablets work if I do not have the JAK2 mutation?

Yes. It blocks the JAK enzymes themselves rather than the mutation, and it helped people with and without JAK2 V617F in the approval studies. Your haematologist prescribes it based on your diagnosis and symptoms.

[3] EMA: Jakavi — European public assessment report

Why am I more prone to shingles and infections?

JAK signalling is also part of the immune system, so infections — particularly shingles, urinary and chest infections — are more common. Tell your team about any fever or a painful one-sided rash promptly; a shingles vaccine before starting is often advised.

Will Jakavi 20 mg Tablets cure my myelofibrosis?

No medicine currently cures myelofibrosis apart from a stem-cell transplant in suitable people. Ruxolitinib controls symptoms and spleen size and is taken long-term. Your haematologist will explain how it fits into your overall plan.

[4] Macmillan Cancer Support: Ruxolitinib (Jakavi)

Do I need a prescription to order Jakavi 20 mg Tablets?

Yes. Ruxolitinib is prescription-only. Upload your haematologist's prescription at checkout; our pharmacist team reviews it before dispatch. Used as prescribed by a licensed medical professional.
