Zidovudine (AZT) was the first antiretroviral drug approved for HIV treatment in 1987. Though largely replaced by better-tolerated NRTIs, it remains important in prevention of mother-to-child HIV transmission (PMTCT) and in some combination regimens in resource-limited settings.

How Does It Work?

Intracellular phosphorylation

Zidovudine is phosphorylated to its active triphosphate form (ZDV-TP) inside host cells, which then competes with the natural substrate.

NRTI action — chain termination

ZDV-TP competes with thymidine triphosphate (TTP) for incorporation into viral DNA by reverse transcriptase, causing premature chain termination.

Suppression of viral replication

Without complete reverse transcription, HIV cannot produce its proviral DNA or establish infection in the target cell.

⚠ BLACK BOX WARNING
