About this medicine

What is Temcad 20 mg Capsules?

Temozolomide is a chemotherapy medicine taken by mouth, used to treat certain brain tumours — most often glioblastoma (a fast-growing tumour of the brain's supporting cells) and anaplastic astrocytoma. For newly diagnosed glioblastoma it is usually given alongside radiotherapy first, then on its own in monthly cycles.

It belongs to a group of medicines called alkylating agents. Unlike many chemotherapy drugs it is a capsule rather than an infusion, so most people take it at home. Because it can cross into the brain it is well suited to tumours there.

Your oncology team will calculate your dose from your height and weight (body surface area) and adjust it according to your blood counts. Temozolomide is a prescription-only medicine; a valid prescription must be provided at checkout.

How it works

How does Temozolomide work?

Cancer cells have to copy their DNA every time they divide. Temozolomide is converted in the body into an active compound that attaches small chemical groups to the DNA inside cells (a process called methylation). This damages the DNA in a way the cell cannot properly repair, so the damaged cell stops dividing and dies. Tumour cells divide far more often than most healthy brain cells, so they are affected the most — though some healthy fast-dividing cells, such as those in the bone marrow, are affected too, which is why blood counts are monitored.

Conversion

After absorption, temozolomide converts spontaneously in the bloodstream to its active form, MTIC. No liver activation is needed.

DNA methylation

MTIC adds methyl groups to the DNA of dividing cells, mainly at the O6 and N7 positions of guanine.

Cell death

The mismatch-repair system cannot fix the damage; the cell stalls and undergoes programmed cell death. Tumours with a silenced MGMT repair gene are less able to reverse the damage.

What is Temozolomide used for?

Newly diagnosed glioblastoma

Given together with radiotherapy, then as maintenance treatment on its own, in adults.

FDA EMA CDSCO
Refractory anaplastic astrocytoma

For adults whose tumour has progressed after treatment containing a nitrosourea and procarbazine.

FDA EMA
Recurrent or progressive high-grade glioma

Adults and children aged three and over, after standard therapy.

Dosage

How to take Temcad 20 mg Capsules

Your oncologist will prescribe your exact dose. The right dose for you depends on your weight, kidney and liver function, and — for some medicines — your genetic test results.

Always take exactly as prescribed. Do not change your dose or stop taking this medicine without speaking to your doctor first, even if you feel well.

Missed a dose? If you miss a dose and remember the same day, take it as soon as you can. If it is nearly time for the next dose, skip the missed one. Never double up. Tell your treatment team if you vomit after a dose — do not take another.

With food? Take on an empty stomach — at least one hour before or after food — with a full glass of water. Swallow capsules whole; do not open, crush or chew them. An anti-sickness tablet is often taken about 30 minutes beforehand.

Important safety information

Safety & warnings

Please read this section before taking Temcad 20 mg Capsules and discuss any concerns with your oncologist.

Common side effects

Side effects affect some people, not everyone. Most are manageable — let your treatment team know about anything that concerns you.

  • Nausea and vomiting
  • Tiredness (fatigue)
  • Constipation
  • Headache
  • Loss of appetite
  • Low blood counts (platelets, white cells)
  • Hair thinning
  • Rash

Contact your doctor immediately if you notice:

  • Fever of 38°C or higher, chills, or signs of infection Go to the emergency department at once. Low white cells make infection dangerous.
  • Unusual bruising, bleeding gums, nosebleeds or blood in urine/stool Contact your treatment team the same day — this can signal a very low platelet count.
  • Breathlessness, dry cough, chest pain Seek urgent review; Pneumocystis pneumonia can occur, especially during the radiotherapy phase, and preventive antibiotics are often prescribed.
  • Yellow skin or eyes, dark urine, severe abdominal pain Contact your doctor immediately — possible liver injury.
  • Seizure, severe headache or new weakness Seek emergency care; these may relate to the tumour or treatment and need assessment.

Important: tell your doctor about these medicines

Taking Temozolomide with certain other medicines can be dangerous. Always tell every doctor, dentist, and pharmacist about all medicines you are taking.

  • Live vaccines (MMR, yellow fever, varicella, BCG) Critical

Temozolomide suppresses the immune system; live vaccines can cause serious infection.

  • Other bone-marrow-suppressing medicines or radiotherapy Major

    Additive lowering of blood counts.

Pregnancy & breastfeeding

Temozolomide can harm an unborn baby and should not be used in pregnancy. Women who could become pregnant should use effective contraception during treatment and for at least six months after the last dose.

Do not breastfeed during treatment or for at least one week after the last dose.

Temozolomide may reduce fertility in men and women. Ask about sperm banking or egg/embryo freezing before your first dose if this matters to you.

Storage

How to store Temcad 20 mg Capsules

Temperature

Store below 25°C (77°F) in the original container.

Light & moisture

Keep the bottle tightly closed and protected from moisture.

Keep out of the sight and reach of children and pets.

Temozolomide is a cytotoxic medicine. Do not open capsules. If a capsule is damaged, avoid contact with skin or eyes and wash any exposed area with water. Return unused capsules to a pharmacy for disposal.

FAQ

Frequently asked questions

Is Temozolomide the same as Temodar?

Temozolomide is the active ingredient in Temodar. A generic Temozolomide medicine contains the same active ingredient at the same strength and is taken the same way — the main difference is cost. Always use it as prescribed by a licensed medical professional.

Is Temcad 20 mg Capsules chemotherapy?

Yes. Temozolomide is a chemotherapy medicine from the alkylating-agent group. It is unusual in being a capsule you take at home rather than an infusion given in hospital.

How is Temcad 20 mg Capsules taken with radiotherapy?

For newly diagnosed glioblastoma it is usually taken every day throughout the course of radiotherapy, then — after a short break — on its own for five days out of every 28-day cycle. Your oncologist will give you a written schedule; follow that rather than any general description.

Why must I take it on an empty stomach?

Food slows and reduces absorption. Taking the capsules at least an hour away from meals, with water, gives more predictable levels. Many people take the dose at bedtime with an anti-sickness tablet 30 minutes before.

What if I am sick after taking Temcad 20 mg Capsules?

Do not take a replacement dose that day. Tell your treatment team — they may adjust your anti-sickness medicine. Take the next dose at the usual time.

Why are my blood counts checked so often?

Temozolomide can lower platelets and white blood cells, usually three to four weeks into a cycle. Your team checks a blood count before each cycle, and more often during radiotherapy, so the next dose can be delayed or reduced if needed.

Is Temcad 20 mg Capsules the same as Temodar?

Temcad 20 mg Capsules contains the same active ingredient, temozolomide, at the same strength as the originator brand Temodar (Temodal in Europe). Generic medicines must meet the same regulatory standards for quality, safety and effectiveness; the difference is price, not the medicine. Used as prescribed by a licensed medical professional.

[3] EMA: Temodal — European public assessment report

Can I handle the capsules safely?

Yes, with normal care: swallow them whole, do not open or crush them, and wash your hands afterwards. Family members should avoid handling broken capsules. Return unused capsules to a pharmacy.

Do I need a prescription to order Temcad 20 mg Capsules?

Yes. Temozolomide is prescription-only everywhere we deliver. Upload your oncologist's prescription at checkout; our pharmacist team reviews it before anything is dispatched. Used as prescribed by a licensed medical professional.
