About this medicine

What is Tafinlar 75 mg Capsules?

Dabrafenib is a targeted capsule for cancers that carry a particular change in the BRAF gene (a 'BRAF V600 mutation') — most often melanoma that has spread or cannot be removed by surgery, and also certain lung, thyroid and other solid tumours, including some in children. It is almost always taken together with trametinib, a MEK inhibitor: the two block consecutive steps of the same growth pathway and work better together than either alone.

Before starting, your tumour must have been tested and shown to carry the BRAF V600 mutation. Dabrafenib is not used for tumours without it, and in BRAF-normal tumours it can even stimulate growth.

Dabrafenib is taken twice a day on an empty stomach. Fever is the most common side effect of the combination and your team will give you a plan for it. It is a prescription-only medicine; a valid prescription must be provided at checkout.

How it works

How does Dabrafenib work?

In BRAF-mutant cancers a faulty BRAF protein sends a permanent 'grow' signal down a chain of proteins called the MAPK pathway (RAS → RAF → MEK → ERK). Dabrafenib blocks the mutant BRAF protein itself, so the signal never starts. Trametinib blocks MEK, the next step down; using both shuts the pathway at two points, which works better and — importantly — prevents the pathway from finding a way round the block, which is what happens when a BRAF inhibitor is used alone. It also reduces the new skin growths that BRAF inhibitors alone can cause in normal skin cells.

The faulty signal

A BRAF V600E or V600K mutation keeps the BRAF kinase permanently active, driving continuous MEK/ERK signalling.

BRAF blockade

Dabrafenib binds the ATP pocket of mutant BRAF and stops it phosphorylating MEK.

Two-point shutdown

With trametinib blocking MEK below, the pathway is closed at two points, delaying resistance and limiting paradoxical activation in BRAF-normal cells.

Dosage

How to take Tafinlar 75 mg Capsules

Your oncologist will prescribe your exact dose. The right dose for you depends on your weight, kidney and liver function, and — for some medicines — your genetic test results.

Always take exactly as prescribed. Do not change your dose or stop taking this medicine without speaking to your doctor first, even if you feel well.

Missed a dose? If it is less than 6 hours until the next dose, skip the missed one. Never take two doses together.

With food? Take on an empty stomach — at least one hour before or two hours after a meal — with a full glass of water. Swallow capsules whole; do not open or chew.

Important safety information

Safety & warnings

Please read this section before taking Tafinlar 75 mg Capsules and discuss any concerns with your oncologist.

Common side effects

Side effects affect some people, not everyone. Most are manageable — let your treatment team know about anything that concerns you.

  • Fever (pyrexia), often with chills — mainly in the first months
  • Tiredness (fatigue)
  • Nausea, vomiting, diarrhoea
  • Headache, joint and muscle pain
  • Rash, dry skin, thickened skin on palms and soles
  • Raised blood sugar
  • Swelling of hands and feet
  • Raised liver enzymes
  • Hair thinning

Contact your doctor immediately if you notice:

  • Fever of 38.5°C or higher, rigors, dizziness or dehydration Contact your team the same day; both drugs are usually paused, the fever treated, and restarted once you are well — a written plan is given at the start.
  • New skin growths, warty lumps or changes to moles Report promptly — new skin cancers (cutaneous squamous cell carcinoma) can develop; skin checks are routine before, during and for six months after treatment.
  • Excessive thirst, passing lots of urine, blurred vision Contact your team — blood sugar can rise, especially in people with diabetes.
  • Blurred vision, seeing halos, eye pain Seek prompt eye assessment — uveitis or retinal problems can occur (the latter mainly from trametinib).
  • Breathlessness, swollen ankles, fast or irregular heartbeat Seek urgent review — reduced heart function or QT prolongation; heart scans and ECG are monitored.
  • Unusual bleeding: coughing or vomiting blood, black stools, severe headache Seek emergency care.
  • Severe abdominal pain, vomiting, fever Seek urgent care — pancreatitis has been reported.
  • Severe blistering rash, or rash with fever and mouth ulcers Stop and contact your team immediately.
  • Very reduced urine, dark urine, muscle pain Seek review — kidney injury and muscle breakdown have been reported, usually alongside fever.

Important: tell your doctor about these medicines

Taking Dabrafenib with certain other medicines can be dangerous. Always tell every doctor, dentist, and pharmacist about all medicines you are taking.

  • Strong CYP3A4 or CYP2C8 inhibitors (ketoconazole, gemfibrozil, clarithromycin, ritonavir) Major

    Increase dabrafenib levels.

  • Strong CYP3A4 or CYP2C8 inducers (rifampicin, phenytoin, carbamazepine, St John's wort) Major

    Reduce dabrafenib levels.

  • Hormonal contraceptives, warfarin, midazolam, dexamethasone and other CYP3A4/CYP2C9 substrates Major

Dabrafenib is a strong inducer and can make them less effective.

Pregnancy & breastfeeding

Dabrafenib can harm an unborn baby. It should not be used in pregnancy, and women who could become pregnant should use effective contraception during treatment and for at least two weeks after the last dose (four months for the trametinib partner).

Do not breastfeed during treatment or for two weeks after the last dose.

Dabrafenib may reduce fertility, particularly sperm production in men. Discuss fertility preservation before starting if relevant.

Storage

How to store Tafinlar 75 mg Capsules

FAQ

Frequently asked questions

Why is Tafinlar 75 mg Capsules given with trametinib?

Dabrafenib blocks BRAF; trametinib blocks MEK, the next step in the same pathway. Together they work better than either alone, resistance takes longer to develop, and the new skin growths seen with dabrafenib alone are much rarer. Dabrafenib on its own is now seldom used.

Do I need a BRAF test before taking Tafinlar 75 mg Capsules?

Yes. Dabrafenib only helps tumours with a BRAF V600 mutation — and in tumours without it, a BRAF inhibitor can actually speed growth. Your oncologist will have confirmed the mutation with a validated test before prescribing.

[1] FDA prescribing information: Tafinlar (dabrafenib) — DailyMed

Is fever normal on this treatment?

Fever affects most people on the dabrafenib–trametinib combination, usually in the first weeks. Your team will give you a plan — typically pausing both drugs, treating the fever, and restarting once you have been well for 24 hours. Do not manage a fever of 38.5°C or more yourself; call your team.

[4] Macmillan Cancer Support: Dabrafenib and trametinib

Why on an empty stomach?

Food — especially a high-fat meal — reduces and delays absorption. Take the capsules at least an hour before or two hours after eating, with water.

Does Tafinlar 75 mg Capsules affect my contraceptive pill?

Yes. Dabrafenib speeds up the breakdown of hormonal contraceptives and can make them unreliable. Use a barrier method or another non-hormonal method throughout treatment and for at least four months after the last dose of trametinib.

Why are my skin and blood sugar checked?

BRAF inhibitors can trigger new skin cancers (usually easily treated squamous cell types) in normal skin, so a dermatology check is done before starting and regularly after. Blood sugar can also rise, so it is monitored — particularly if you have diabetes.

Do I need a prescription to order Tafinlar 75 mg Capsules?

Yes. Dabrafenib is prescription-only. Upload your oncologist's prescription at checkout; our pharmacist team reviews it before dispatch. Used as prescribed by a licensed medical professional.
