Overview

What is Basiliximab?

Basiliximab is a chimeric monoclonal antibody given as just two intravenous doses at the time of kidney transplant (day 0 and day 4). It blocks the IL-2 receptor on activated T cells during the highest-risk period for acute rejection. Unlike maintenance immunosuppressants, it is not taken long-term.
Mechanism

How Does It Work?

1
IL-2 receptor binding

Basiliximab binds the CD25 alpha subunit of the high-affinity IL-2 receptor expressed on activated T lymphocytes, blocking IL-2 from docking.

2
Clonal expansion blockade

Without IL-2 signalling, antigen-specific T cells cannot undergo the clonal proliferation that drives acute rejection of the transplanted organ.

3
Receptor saturation period

Therapeutic saturation of CD25 receptors is maintained for 4–6 weeks after two doses, covering the highest-risk period for acute rejection after surgery.
