About this medicine

What is Imbruvica 140 mg Capsules?

Ibrutinib is a targeted medicine taken by mouth for blood cancers that start in B lymphocytes — chronic lymphocytic leukaemia (CLL) and small lymphocytic lymphoma, Waldenström macroglobulinaemia, and in some regions mantle cell and marginal zone lymphoma. It is also used for chronic graft-versus-host disease after a stem-cell transplant when steroids have not worked.

It was the first medicine to block Bruton tyrosine kinase (BTK), an enzyme the cancerous B cells depend on to survive. It is taken once a day, continuously, for as long as it keeps working and is tolerated — often for years.

Bleeding, irregular heart rhythm (atrial fibrillation), high blood pressure and infections are the side effects your haematologist watches for. Ibrutinib is a prescription-only medicine; a valid prescription must be provided at checkout.

How it works

How does Ibrutinib work?

B cells carry a receptor that tells them when to grow and survive; inside the cell that message passes through an enzyme called BTK. In CLL and related cancers the signal is stuck on. Ibrutinib binds permanently to BTK and switches it off, so the survival message stops and the malignant B cells gradually die. It also stops them clinging to lymph nodes, which is why the lymphocyte count in the blood often rises for a few weeks after starting — an expected effect, not a sign of failure. Ibrutinib also blocks some other enzymes, which accounts for its heart-rhythm and bleeding side effects; newer BTK inhibitors are more selective.

Covalent binding

Ibrutinib forms an irreversible bond with cysteine-481 in the BTK active site.

Signal cut off

With BTK inactive, the B-cell receptor cannot transmit its survival and proliferation signal.

Release and death

Malignant B cells lose their anchorage in lymph nodes and their survival signal — they circulate briefly (lymphocytosis) and die.

Dosage

How to take Imbruvica 140 mg Capsules

Your oncologist will prescribe your exact dose. The right dose for you depends on your weight, kidney and liver function, and — for some medicines — your genetic test results.

Always take exactly as prescribed. Do not change your dose or stop taking this medicine without speaking to your doctor first, even if you feel well.

Missed a dose? Take the missed dose as soon as you remember the same day, then return to the usual time. Do not take two doses in one day.

With food? Take with or without food, with a glass of water. Swallow capsules whole; do not open or chew. Avoid grapefruit and Seville oranges.

Important safety information

Safety & warnings

Please read this section before taking Imbruvica 140 mg Capsules and discuss any concerns with your oncologist.

Common side effects

Side effects affect some people, not everyone. Most are manageable — let your treatment team know about anything that concerns you.

  • Diarrhoea
  • Bruising and minor bleeding
  • Muscle and joint pain, muscle spasms
  • Tiredness (fatigue)
  • Rash
  • Upper respiratory infections, pneumonia
  • Low blood counts (neutrophils, platelets)
  • Raised blood pressure
  • Nausea, mouth soreness
  • Brittle nails, hair changes

Contact your doctor immediately if you notice:

  • Palpitations, fluttering heartbeat, dizziness or fainting Seek urgent review — atrial fibrillation and other arrhythmias, including rare serious ones, are a known risk.
  • Bleeding that will not stop, blood in urine or stool, severe headache Seek emergency care — serious haemorrhage is uncommon but can occur, especially with blood thinners.
  • Fever, chills, cough or other signs of infection Contact your team the same day; serious infections including fungal infections and hepatitis B reactivation can occur.
  • Very high blood pressure, severe headache Seek review — blood pressure often rises and needs treatment.
  • Breathlessness, swollen ankles, chest pain Seek urgent care — heart failure has been reported.
  • New skin lumps or changes in a mole Report at your next visit — other skin cancers are seen more often; use sun protection.
  • Very reduced urine, muscle cramps, palpitations soon after starting Seek urgent care — tumour lysis syndrome is rare but possible.
  • Planned surgery or dental extraction Tell your haematologist — ibrutinib is usually paused 3–7 days before and after procedures.

Important: tell your doctor about these medicines

Taking Ibrutinib with certain other medicines can be dangerous. Always tell every doctor, dentist, and pharmacist about all medicines you are taking.

  • Strong CYP3A inhibitors (ketoconazole, itraconazole, clarithromycin, ritonavir) and grapefruit Critical

    Increase ibrutinib levels many-fold.

  • Moderate CYP3A inhibitors (fluconazole, diltiazem, verapamil, erythromycin) Major

    Increase ibrutinib levels.

  • Strong CYP3A inducers (rifampicin, phenytoin, carbamazepine, St John's wort) Major

Lower ibrutinib levels and may make it ineffective.

  • Anticoagulants and antiplatelet medicines (warfarin, apixaban, aspirin, clopidogrel) Major

    Additive bleeding risk.

  • Live vaccines Major

Safety not established; immune response may be reduced.

Pregnancy & breastfeeding

Ibrutinib may harm an unborn baby. It should be avoided in pregnancy, and women who could become pregnant should use effective contraception during treatment and for at least three months after the last dose.

Do not breastfeed during treatment or for one week after the last dose.

Effects on human fertility are not established. Discuss fertility preservation before starting if relevant.

FAQ

Frequently asked questions

Is Imbruvica 140 mg Capsules chemotherapy?

No. Ibrutinib is a targeted therapy that blocks one enzyme (BTK) inside cancerous B cells. Hair loss and severe sickness are uncommon; its own risks are bleeding, heart rhythm and infection, which your haematologist monitors.

[2] NCI Drug Information: Ibrutinib

My lymphocyte count went up after starting — is it working?

A temporary rise in blood lymphocytes in the first weeks is expected: ibrutinib releases cancer cells from lymph nodes into the bloodstream, where they then die off. Your team looks at node size, spleen and other counts, not that number alone.

[1] FDA prescribing information: Imbruvica (ibrutinib) — DailyMed

How does Imbruvica 140 mg Capsules compare with acalabrutinib or zanubrutinib?

They all block BTK. Ibrutinib was first and has the longest track record; the newer drugs are more selective and in head-to-head studies caused fewer heart-rhythm problems and less high blood pressure at similar effectiveness. Your haematologist chooses based on your heart history, other medicines and availability.

[3] EMA: Imbruvica — European public assessment report

Why must I avoid grapefruit?

Grapefruit and Seville oranges block the enzyme (CYP3A) that clears ibrutinib and can raise its level several-fold. Other citrus is fine.

[1] FDA prescribing information: Imbruvica (ibrutinib) — DailyMed

Can I take aspirin or ibuprofen?

Only if your haematologist agrees. Ibrutinib itself increases bleeding tendency, and aspirin, anti-inflammatories and blood thinners add to it. Paracetamol is the usual choice for pain.

[4] Macmillan Cancer Support: Ibrutinib (Imbruvica)

How long will I take Imbruvica 140 mg Capsules?

Usually continuously for as long as it keeps the disease controlled and is tolerated — often years. Do not stop or pause it yourself; if a pause is needed for surgery or an infection, your haematologist will tell you when and for how long.

[1] FDA prescribing information: Imbruvica (ibrutinib) — DailyMed

Do I need a prescription to order Imbruvica 140 mg Capsules?

Yes. Ibrutinib is prescription-only. Upload your haematologist's prescription at checkout; our pharmacist team reviews it before dispatch. Used as prescribed by a licensed medical professional.
