How Does It Work?
Sexual stimulation or pulmonary vasoconstriction triggers nitric oxide (NO) release, which activates guanylate cyclase to increase cyclic GMP (cGMP), causing smooth muscle relaxation and vasodilation.
Sildenafil blocks phosphodiesterase type 5 (PDE5), the enzyme that degrades cGMP in smooth muscle of the corpus cavernosum and pulmonary vasculature, prolonging and enhancing the vasodilatory effect.
In the penis this increased blood flow produces erection; in the pulmonary vasculature it reduces pulmonary vascular resistance, lowering right heart afterload in PAH.
How to Use
- ED: 50 mg approximately 1 hour before sexual activity (range 25–100 mg); maximum one dose per day
- PAH (Revatio): 20 mg three times daily, approximately 4–6 hours apart
- High-fat meal delays ED absorption by up to 60 minutes — take on empty stomach for fastest onset
