About this medicine

What is Cosalon 50 mg Tablets?

Bicalutamide is a hormone-therapy tablet for prostate cancer. Prostate cancer cells are fuelled by testosterone; bicalutamide sits on the cells' androgen receptor and blocks the hormone from switching it on. It is most often taken at 50 mg a day together with an LHRH agonist injection (such as goserelin or leuprorelin) — the injection stops the testicles making testosterone and bicalutamide blocks whatever remains. It is also given for the first few weeks after an LHRH injection to prevent the temporary testosterone surge ('flare') the injection causes.

At a higher 150 mg dose it is used on its own in some countries for locally advanced disease, which keeps testosterone levels normal and so causes fewer hot flushes but more breast tenderness.

Bicalutamide is one of the older, well-established anti-androgens and is inexpensive. It is taken once a day with or without food. It is a prescription-only medicine; a valid prescription must be provided at checkout.

How it works

How does Bicalutamide work?

Testosterone drives prostate cancer by attaching to a receptor inside the cancer cell and switching on growth genes. Bicalutamide is shaped to fit the same receptor but does not switch it on — it occupies the site and keeps testosterone out. With the receptor blocked, the growth signal falls and the cancer slows. It does not lower testosterone itself; that is what the LHRH injection does, which is why the two are usually used together (combined androgen blockade). When an LHRH agonist is first started it briefly raises testosterone before suppressing it, and bicalutamide taken over those weeks prevents that surge from stimulating the tumour.

Receptor competition

Bicalutamide binds the androgen receptor's ligand pocket competitively, without activating it.

Signal blocked

Androgen-driven transcription of growth and survival genes in prostate cells falls.

With LHRH agonist

Castration removes testicular testosterone; bicalutamide blocks adrenal androgens and covers the initial flare.

Dosage

How to take Cosalon 50 mg Tablets

Your oncologist will prescribe your exact dose. The right dose for you depends on your weight, kidney and liver function, and — for some medicines — your genetic test results.

Always take exactly as prescribed. Do not change your dose or stop taking this medicine without speaking to your doctor first, even if you feel well.

Missed a dose? Take it as soon as you remember unless it is nearly time for the next dose; never double up.

With food? With or without food. Swallow whole with water.

Important safety information

Safety & warnings

Please read this section before taking Cosalon 50 mg Tablets and discuss any concerns with your oncologist.

Common side effects

Side effects affect some people, not everyone. Most are manageable — let your treatment team know about anything that concerns you.

  • Breast tenderness and enlargement (gynaecomastia)
  • Hot flushes
  • Tiredness, weakness
  • Reduced libido, erectile difficulty
  • Anaemia
  • Dizziness, constipation, nausea
  • Raised liver enzymes
  • Weight gain, fluid retention
  • Rash, itching, hair thinning or regrowth

Contact your doctor immediately if you notice:

  • Yellow skin or eyes, dark urine, severe tiredness, pain under the right ribs Stop and contact your doctor immediately — rare but serious liver injury has occurred, usually in the first months.
  • New breathlessness or dry cough Seek prompt review — lung inflammation (interstitial lung disease) is rare but reported.
  • Chest pain, palpitations, fainting Seek urgent care — QT prolongation and cardiovascular events are possible with androgen-deprivation therapy.
  • Unusual bleeding or bruising while on warfarin Contact your team — INR can rise.
  • Sudden severe bone pain, or a fall with a possible fracture Seek assessment — bone density falls on long-term hormone therapy.

Important: tell your doctor about these medicines

Taking Bicalutamide with certain other medicines can be dangerous. Always tell every doctor, dentist, and pharmacist about all medicines you are taking.

  • Warfarin Major

Bicalutamide displaces warfarin from plasma proteins and can raise INR.

  • Medicines that prolong the QT interval (some antiarrhythmics, antipsychotics, antibiotics) Major

Additive heart-rhythm risk with androgen deprivation.

Pregnancy & breastfeeding

Bicalutamide is not indicated for women and can harm an unborn baby by blocking androgen action in a male fetus.

Not applicable — not for use in women.

Bicalutamide reduces sperm production while taken. Discuss sperm banking before starting if this matters to you.

FAQ

Frequently asked questions

Why do I take Cosalon 50 mg Tablets as well as my hormone injection?

The injection stops the testicles making testosterone; bicalutamide blocks the small amount still produced by the adrenal glands from acting on the cancer, and it prevents the temporary testosterone surge the first injection causes. Together this is called combined androgen blockade. Some men take bicalutamide only for the first few weeks; others continue it long-term — follow your specialist's plan.

Is Cosalon 50 mg Tablets chemotherapy?

No. It is a hormone therapy that blocks testosterone's effect on the cancer. Hair loss, sickness and low blood counts are not typical; hot flushes, tiredness, breast tenderness and reduced libido are the usual effects.

[2] NCI Drug Information: Bicalutamide

What can be done about breast tenderness and swelling?

Gynaecomastia is the most common side effect, especially at the 150 mg dose. A short course of low-dose radiotherapy to the breast tissue before starting, or tamoxifen alongside, reduces it substantially — ask your specialist. It is not dangerous, but it can be uncomfortable and it may not fully reverse after long treatment.

How does Cosalon 50 mg Tablets compare with enzalutamide or apalutamide?

They all block the androgen receptor. The newer drugs bind it far more strongly and also stop it moving into the cell nucleus, so they work in cancers that have become resistant to bicalutamide — at a much higher cost. Bicalutamide remains standard for flare prevention and for combined blockade in hormone-sensitive disease. Your specialist chooses based on the stage of your cancer.

[2] NCI Drug Information: Bicalutamide

Will Cosalon 50 mg Tablets affect my sex life?

Often, yes — reduced libido and erectile difficulty are common with any treatment that blocks testosterone, more so when combined with an LHRH injection. Erectile-dysfunction medicines can still be used; discuss options with your specialist rather than stopping treatment.

[4] Macmillan Cancer Support: Bicalutamide (Casodex)

Is Cosalon 50 mg Tablets the same as Casodex?

Cosalon 50 mg Tablets contains the same active ingredient, bicalutamide, at the same 50 mg strength as the originator brand Casodex. Generic medicines must meet the same regulatory standards for quality, safety and effectiveness. Used as prescribed by a licensed medical professional.

[3] Electronic Medicines Compendium (UK): Bicalutamide SmPC

Why are liver tests done?

Bicalutamide occasionally causes liver inflammation, usually within the first six months and usually mild. Periodic liver blood tests catch it early; report yellowing of the skin or eyes, dark urine or persistent nausea between tests.

Do I need a prescription to order Cosalon 50 mg Tablets?

Yes. Bicalutamide is prescription-only. Upload your specialist's prescription at checkout; our pharmacist team reviews it before dispatch. Used as prescribed by a licensed medical professional.
