About this medicine

What is Azadine 100 mg Injection?

Azacitidine is a medicine for blood cancers that start in the bone marrow — myelodysplastic syndromes (MDS), acute myeloid leukaemia (AML) in people who cannot have intensive chemotherapy, and chronic myelomonocytic leukaemia. It works differently from most chemotherapy: at low doses it switches back on genes that the cancer has silenced, helping abnormal marrow cells mature normally.

It comes in two very different forms. The injection (100 mg vials) is given under the skin or into a vein by trained staff, typically for seven days of each 28-day cycle, and is used for MDS, AML and CMML — often with venetoclax in AML. The tablet (300 mg, the Onureg presentation) is taken at home for 14 days of each cycle and is used only as maintenance in AML after chemotherapy has produced a remission. The doses are not interchangeable and the tablet must not be used in place of the injection.

Low blood counts, nausea and injection-site reactions are the main side effects. Azacitidine is a prescription-only medicine; a valid prescription must be provided at checkout, and the injection must be administered by a healthcare professional.

How it works

How does Azacitidine work?

Genes can be switched off by small chemical tags (methyl groups) on the DNA. In MDS and AML the cancer cells have silenced the genes that would normally tell them to mature and stop dividing. Azacitidine is a fake building block that is taken into the DNA of dividing cells and traps the enzyme that adds those tags, so with each cell division the tags are lost and the silenced genes switch back on — the cells can differentiate and die on schedule. At higher concentrations it also damages RNA and DNA directly. It does not cure the underlying condition, but it can control it for a long time and reduce the need for transfusions.

Incorporation

Azacitidine is phosphorylated and incorporated into RNA and, after conversion to decitabine triphosphate, into DNA of dividing cells.

DNMT trapping

DNA methyltransferase binds the azacytosine base irreversibly and is degraded, so daughter DNA is progressively demethylated.

Re-expression

Silenced tumour-suppressor and differentiation genes are re-expressed; at higher doses direct cytotoxicity adds to the effect.

Dosage

How to take Azadine 100 mg Injection

Your oncologist will prescribe your exact dose. The right dose for you depends on your weight, kidney and liver function, and — for some medicines — your genetic test results.

Always take exactly as prescribed. Do not change your dose or stop taking this medicine without speaking to your doctor first, even if you feel well.

Missed a dose? Injection: contact the clinic to reschedule; the cycle is not restarted. Tablets: take the missed dose as soon as possible on the same day, then resume the usual schedule the next day; never take two doses in one day; tell your team if you vomit.

With food? Injection: not applicable; anti-sickness medicine is given beforehand. Tablets: with or without food; swallow whole — do not crush, split or chew. Anti-sickness medicine is usually taken 30 minutes before for the first two cycles.

Important safety information

Safety & warnings

Please read this section before taking Azadine 100 mg Injection and discuss any concerns with your oncologist.

Common side effects

Side effects affect some people, not everyone. Most are manageable — let your treatment team know about anything that concerns you.

  • Low blood counts (neutrophils, platelets, red cells)
  • Nausea, vomiting, diarrhoea, constipation
  • Injection-site redness, pain, bruising (injection)
  • Tiredness (fatigue), weakness
  • Fever, chest and urinary infections
  • Reduced appetite, weight loss
  • Joint and muscle pain
  • Rash, itching
  • Dizziness, headache, sleep disturbance
  • Low potassium

Contact your doctor immediately if you notice:

  • Fever of 38°C or higher, chills, or signs of infection Go to the emergency department — neutropenic infection is a medical emergency.
  • Unusual bruising, bleeding gums, nosebleeds, blood in urine or stool Contact your team the same day — possible very low platelets.
  • Severe tiredness, breathlessness, pale skin Contact your team; transfusion may be needed.
  • Very reduced urine, muscle cramps, palpitations early in treatment Seek urgent care — tumour lysis syndrome can occur when there is a high cell count.
  • Severe abdominal pain, bloody diarrhoea Seek emergency care — necrotising bowel inflammation has been reported rarely.
  • Yellow skin or eyes, dark urine Contact your doctor immediately — liver toxicity is more likely with existing liver disease.
  • New breathlessness or dry cough Seek urgent review — lung inflammation has been reported.

Important: tell your doctor about these medicines

Taking Azacitidine with certain other medicines can be dangerous. Always tell every doctor, dentist, and pharmacist about all medicines you are taking.

  • Other bone-marrow-suppressing medicines Major

    Additive lowering of blood counts.

  • Live vaccines Critical

Risk of serious infection while immunosuppressed.

  • Oral azacitidine substituted for the injection (or vice versa) Critical

The doses and exposures differ several-fold; substitution has caused serious harm.

Pregnancy & breastfeeding

Azacitidine can harm an unborn baby and should not be used in pregnancy. Women who could become pregnant should use effective contraception during treatment and for at least six months after the last dose.

Do not breastfeed during treatment or for one week after the last dose.

Azacitidine may impair fertility in men and women. Discuss sperm banking or egg/embryo freezing before your first dose if relevant.

FAQ

Frequently asked questions

Is Azacitidine the same as Vidaza?

Azacitidine is the active ingredient in Vidaza. A generic Azacitidine medicine contains the same active ingredient at the same strength and is taken the same way — the main difference is cost. Always use it as prescribed by a licensed medical professional.

Are the injection and the tablets interchangeable?

No — and this matters. The tablet (300 mg, Onureg-type) delivers far less drug than the injection and is approved only as maintenance after AML remission; the injection is the treatment for MDS, AML and CMML. Using one in place of the other has caused serious harm. Always check which form your haematologist prescribed.

Is Azadine 100 mg Injection chemotherapy?

It is classed as chemotherapy, but at the doses used it works mainly by switching silenced genes back on ("hypomethylation") rather than by killing cells outright. Hair loss is uncommon; low blood counts, nausea and tiredness are the usual effects.

[3] NCI Drug Information: Azacitidine

How long before Azadine 100 mg Injection works?

Slowly. Blood counts often fall in the first cycles before they improve, and a response is usually judged after four to six cycles. Stopping early because counts have dropped is a common mistake — discuss any concerns with your haematologist rather than pausing yourself.

[5] Macmillan Cancer Support: Azacitidine (Vidaza)

Can I have the injection at home?

It is normally given in a day unit by trained staff, seven days in a row (or five days, a weekend break, then two). Some centres arrange home administration by a nurse. It is not self-injected.

Why the injection-site lumps and bruises?

Under-the-skin injections of azacitidine commonly cause redness, swelling or a bruise where they are given. Sites are rotated between the abdomen, thighs and upper arms. Tell the nurse if a site becomes very painful, hot or discharges.

[5] Macmillan Cancer Support: Azacitidine (Vidaza)

Is Azadine 100 mg Injection the same as Vidaza / Onureg?

Azadine 100 mg Injection contains the same active ingredient, azacitidine, at the same strength as the corresponding originator product (Vidaza injection or Onureg tablets). Generic medicines must meet the same regulatory standards for quality, safety and effectiveness. Used as prescribed by a licensed medical professional.

[4] EMA: Vidaza — European public assessment report

Will Azadine 100 mg Injection cure my MDS?

No — only a stem-cell transplant can cure MDS or AML in suitable people. Azacitidine controls the disease, improves counts and reduces transfusions for as long as it keeps working, and is sometimes used to prepare for a transplant. Your haematologist will explain how it fits your plan.

[5] Macmillan Cancer Support: Azacitidine (Vidaza)

Do I need a prescription to order Azadine 100 mg Injection?

Yes. Azacitidine is prescription-only, and the injection is for professional administration. Upload your haematologist's prescription at checkout; our pharmacist team reviews it and, for the injection, confirms delivery arrangements with your clinic before dispatch. Used as prescribed by a licensed medical professional.
