About this medicine

What is Acaluxen 100 mg Capsules?

Acalabrutinib is a targeted medicine taken by mouth for certain blood cancers that start in B lymphocytes — chronic lymphocytic leukaemia (CLL) and small lymphocytic lymphoma (SLL), and mantle cell lymphoma (MCL) that has come back or not responded to earlier treatment. It may be given on its own or with an anti-CD20 antibody such as obinutuzumab.

It is a 'BTK inhibitor': it blocks an enzyme the cancerous B cells depend on to survive and multiply. Compared with the first drug of this kind, ibrutinib, it is more selective, which for many people means fewer heart-rhythm and bleeding side effects — although those risks still exist and are monitored.

Acalabrutinib is taken twice a day, about 12 hours apart, continuously for as long as it is working and tolerated. Your haematologist sets the dose. It is a prescription-only medicine; a valid prescription must be provided at checkout.

How it works

How does Acalabrutinib work?

B cells carry a receptor on their surface that tells them when to grow and survive. Inside the cell that message passes through an enzyme called Bruton tyrosine kinase (BTK). In CLL and mantle cell lymphoma the signal is stuck on, keeping the abnormal cells alive. Acalabrutinib binds permanently to BTK and switches it off, so the survival message stops and the cancerous B cells gradually die. Early in treatment the number of lymphocytes in the blood can rise temporarily as cells leave the lymph nodes — this is expected and is not a sign that the drug is failing.

Covalent binding

Acalabrutinib forms an irreversible bond with a cysteine in the BTK active site, blocking it until the cell makes new enzyme.

Signal cut off

With BTK inactive, the B-cell receptor can no longer transmit the survival and proliferation signal downstream.

Cell death and release

Malignant B cells lose their survival signal and their ability to stay anchored in lymph nodes, so they circulate briefly (lymphocytosis) and then die.

Dosage

How to take Acaluxen 100 mg Capsules

Your oncologist will prescribe your exact dose. The right dose for you depends on your weight, kidney and liver function, and — for some medicines — your genetic test results.

Always take exactly as prescribed. Do not change your dose or stop taking this medicine without speaking to your doctor first, even if you feel well.

Missed a dose? If more than three hours have passed, skip the missed dose and take the next one at the usual time. Do not take two doses together.

With food? Take with or without food. Swallow whole with water; do not open, crush or chew. The maleate tablet formulation can be taken with acid-reducing medicines; the older capsule must not be taken with proton-pump inhibitors — check which you have.

Important safety information

Safety & warnings

Please read this section before taking Acaluxen 100 mg Capsules and discuss any concerns with your oncologist.

Common side effects

Side effects affect some people, not everyone. Most are manageable — let your treatment team know about anything that concerns you.

  • Headache (especially in the first weeks)
  • Diarrhoea
  • Bruising and minor bleeding
  • Muscle and joint aches
  • Tiredness (fatigue)
  • Upper respiratory infections
  • Low blood counts (neutrophils, platelets, haemoglobin)
  • Rash
  • Raised blood pressure

Contact your doctor immediately if you notice:

  • Fever, chills, cough or other signs of infection Contact your team the same day; serious infections (including pneumonia and hepatitis B reactivation) can occur.
  • Bleeding that will not stop, blood in urine or stool, severe headache Seek emergency care — serious haemorrhage is rare but possible, especially with blood thinners.
  • Palpitations, fluttering heartbeat, dizziness or fainting Seek urgent review — possible atrial fibrillation or another arrhythmia.
  • Very frequent or severe headache that does not respond to paracetamol Tell your team; headache usually settles within weeks but should be checked.
  • New skin lumps or changes in a mole Report at your next visit — other skin cancers are seen more often on BTK inhibitors; use sun protection.
  • Planned surgery or dental extraction Tell your haematologist in advance — acalabrutinib is usually paused around procedures because of bleeding risk.

Important: tell your doctor about these medicines

Taking Acalabrutinib with certain other medicines can be dangerous. Always tell every doctor, dentist, and pharmacist about all medicines you are taking.

  • Strong CYP3A inhibitors (ketoconazole, itraconazole, clarithromycin, ritonavir) Major

    Greatly increase acalabrutinib levels.

  • Strong CYP3A inducers (rifampicin, phenytoin, carbamazepine, St John's wort) Major

Lower acalabrutinib levels and may make it ineffective.

  • Proton-pump inhibitors (omeprazole, pantoprazole) — capsule formulation only Major

Reduce absorption of the capsule form. The maleate tablet is not affected.

  • Anticoagulants and antiplatelet medicines (warfarin, apixaban, aspirin, clopidogrel) Major

    Additive bleeding risk.

  • Live vaccines Major

Safety not established; immune response may be reduced.

Pregnancy & breastfeeding

Acalabrutinib may harm an unborn baby based on animal studies. It should be avoided in pregnancy, and women who could become pregnant should use effective contraception during treatment and for at least one week after the last dose.

Do not breastfeed during treatment or for two days after the last dose.

Effects on human fertility are not established.

FAQ

Frequently asked questions

Is Acalabrutinib the same as Calquence?

Acalabrutinib is the active ingredient in Calquence. A generic Acalabrutinib medicine contains the same active ingredient at the same strength and is taken the same way — the main difference is cost. Always use it as prescribed by a licensed medical professional.

Is Acaluxen 100 mg Capsules chemotherapy?

No. Acalabrutinib is a targeted therapy that blocks one enzyme (BTK) inside cancerous B cells. It does not damage all dividing cells the way chemotherapy does, so hair loss and severe sickness are uncommon.

[2] NCI Drug Information: Acalabrutinib

How is Acaluxen 100 mg Capsules different from ibrutinib?

Both block BTK. Acalabrutinib is more selective — it binds fewer other enzymes — which in comparative studies meant fewer heart-rhythm problems, less high blood pressure and less joint pain, at similar effectiveness. The risks are lower, not zero, so monitoring continues. Your haematologist chooses between them for your situation.

[3] EMA: Calquence — European public assessment report

My lymphocyte count went up after starting — is it working?

A temporary rise in blood lymphocytes in the first weeks is expected with BTK inhibitors: the drug releases cancer cells from lymph nodes into the bloodstream, where they then die off. Your team looks at the whole picture — node size, spleen, other blood counts — not that number alone.

Can I take Acaluxen 100 mg Capsules with my stomach tablet (omeprazole)?

It depends on the formulation. The original capsule needs stomach acid to be absorbed, so proton-pump inhibitors must be avoided with it. The newer maleate tablet does not have this problem. Check with your pharmacist which one you have been supplied.

Why the headaches?

Headache is the most common early side effect and usually eases within a few weeks. Paracetamol and caffeine often help. Avoid regular aspirin or ibuprofen unless your team agrees, because of the bleeding risk. Tell your team if headaches are severe or persistent.

[4] Macmillan Cancer Support: Acalabrutinib

Is Acaluxen 100 mg Capsules the same as Calquence?

Acaluxen 100 mg Capsules contains the same active ingredient, acalabrutinib, at the same 100 mg strength as the originator brand Calquence. Generic medicines must meet the same regulatory standards for quality, safety and effectiveness. Used as prescribed by a licensed medical professional.

[3] EMA: Calquence — European public assessment report

How long will I take Acaluxen 100 mg Capsules?

It is taken continuously — usually for as long as it keeps the disease controlled and the side effects are acceptable, which can be years. Do not stop or pause it yourself; stopping suddenly can let the disease flare.

Do I need a prescription to order Acaluxen 100 mg Capsules?

Yes. Acalabrutinib is prescription-only. Upload your haematologist's prescription at checkout; our pharmacist team reviews it before dispatch. Used as prescribed by a licensed medical professional.
