“How much is Biktarvy without insurance?” is one of the most searched questions about HIV treatment, and the honest answer is: a lot, at full price. But the full price is not what most people pay. Depending on where you live, HIV treatment can cost you nothing, a small fixed charge, or a co-payment, and licensed generic versions exist in many countries.

This guide gives dated prices from official sources, explains how people actually pay in the US, UK and Australia, and covers where generics fit. Prices change, so treat every figure here as a snapshot.

What brand HIV medicines cost in the US

US drug prices are usually quoted in two ways. The list price (wholesale acquisition cost) is what the manufacturer charges wholesalers. The average wholesale price (AWP) is a benchmark that, in the data below, is calculated as the list price plus 20%. Neither includes insurance, rebates or discounts.

Gilead’s price disclosure for Vermont prescribers, using First Databank data from 30 August 2026, gives these AWPs for a 30-tablet pack, one month’s supply1:

Medicine AWP, 30 tablets (30 Aug 2026) Approximate list price (before the 20% mark-up)
Biktarvy $5,059.32 about $4,216
Genvoya $5,307.23 about $4,423
Triumeq $4,817.21 about $4,014
Dovato $3,900.56 about $3,250
Tivicay (dolutegravir alone) $2,873.89 about $2,395

At list price, a year of Biktarvy comes to over $50,000. A pharmacy’s cash price for someone paying out of pocket can be different again, and is often higher than list.

One change is on the way. In January 2026, Medicare chose Biktarvy as one of 15 medicines for its third round of price negotiation. Negotiations take place during 2026, and any negotiated price takes effect in 20282. That price applies to Medicare, not to everyone.

Why HIV medicines cost so much

Newer HIV medicines are protected by patents, so for many years only the company that developed them can sell them in countries where those patents apply. DailyMed, the US register of medicine labels, lists no generic version of Biktarvy, Dovato or Triumeq as of October 202689. The price also reflects that HIV treatment is taken every day for life, and that the US sets drug prices very differently from countries with national health services.

How people actually pay in the US

Very few people in the US pay the full list price for HIV treatment. HIV.gov lists the main routes3:

  • Private insurance. Most plans cover HIV treatment. Under the Affordable Care Act, plans cannot refuse cover because of HIV, though you still pay deductibles, co-payments and coinsurance.
  • Medicaid and Medicare, for people who qualify.
  • The Ryan White HIV/AIDS Program, which provides care, medicines and support to low-income people with HIV. Over half of people with diagnosed HIV in the US receive services through it each year. Its AIDS Drug Assistance Program (ADAP) provides HIV medicines for people with limited or no insurance.
  • Manufacturer programs. Patient assistance programs give free or reduced-cost medicine to people without adequate insurance who do not qualify for federal help. Co-payment assistance programs (such as copay cards) help insured people pay less towards their share.

If cost is a problem, ask your HIV clinic for a case manager or benefits counsellor. Finding the right program is often the quickest fix.

United Kingdom: free on the NHS

In England, the diagnosis and treatment of HIV is one of the NHS services that is free for everyone, including overseas visitors who would otherwise be charged for NHS care4. Treatment is provided through NHS HIV clinics. If you are in the UK and paying for HIV medicine, it is worth contacting a local NHS sexual health or HIV clinic first.

Australia: the PBS charge

In Australia, HIV medicines are subsidised through the Pharmaceutical Benefits Scheme (PBS) under the Highly Specialised Drugs program. Biktarvy, for example, is PBS item 11649D. A prescription can cover up to 60 tablets (two months), with up to five repeats. The full PBS price for that quantity is listed at $1,833.44, but the general patient charge is $25.005, and the concession charge is $7.706.

People who cannot use the PBS, such as some visitors and temporary visa holders, can face much higher costs. Our guide to importing prescription medicine to Australia explains the personal importation rules.

Licensed generic HIV medicines

Generic versions of many HIV medicines are made in India and used widely around the world. For newer medicines this mostly happens under voluntary licences. For example, in 2017 Gilead licensed bictegravir (the main ingredient in Biktarvy) to the Medicines Patent Pool, a UN-backed organisation. The licence lets approved manufacturers in India, China and South Africa make bictegravir medicines to WHO, European or US quality standards, for sale in 117 low- and middle-income countries7.

Two points follow from this:

  • The ingredients are the same. A licensed generic of Biktarvy contains bictegravir, emtricitabine and tenofovir alafenamide in the same doses.
  • Where you can legally get it depends on where you live. These licences cover sale in specific countries. Bringing a generic into a country such as the US, UK or Australia for personal use is governed by that country’s own import rules. Read our guides for the USA, the UK and Australia before ordering.

What generics cost at OnlineMeds

As of 14 October 2026, a 30-tablet pack of generic bictegravir, emtricitabine and tenofovir alafenamide (the same three ingredients as Biktarvy) costs $110 as Taftrio and $120 as Taffic. Current prices for the other combinations are on our pages for generic Biktarvy, generic Dovato and generic Triumeq. A valid prescription is required, and any switch between brands, or from brand to generic, should be agreed with your HIV team.

Keeping costs down without risking your treatment

  • Never stretch a supply by skipping doses. Missed doses are the main way HIV becomes resistant to treatment, which is far more costly in the long run. Our guide to drug resistance explains why.
  • Order early. Reorder at least a week before you run out, especially if your medicine comes from abroad.
  • Use the programs you qualify for. In the US, check ADAP and manufacturer programs. In the UK and Australia, use the NHS and the PBS where you can.
  • Ask about all your options. Your HIV team may be able to suggest a regimen that is equally suitable for you and easier to get. Our list of HIV medication names shows the brand and generic names of each medicine.

Frequently asked questions

How much is Biktarvy without insurance?

The US average wholesale price was $5,059.32 for 30 tablets on 30 August 2026, which puts the manufacturer’s list price at about $4,216 a month. Most people pay far less through insurance, Medicaid, Medicare, ADAP or Gilead’s assistance programs.

How much is Dovato per month?

The US average wholesale price was $3,900.56 for 30 tablets on 30 August 2026, about $3,250 at list price. As with Biktarvy, insurance and assistance programs usually reduce what people pay.

Is HIV treatment free?

In England it is free on the NHS for everyone, including overseas visitors. In Australia, people who can use the PBS pay a fixed charge per prescription. In the US it depends on insurance, but programs such as ADAP can provide medicines at no cost to people who qualify.

Why is Biktarvy so expensive?

It is a newer medicine still under patent, so there is no US generic, and US list prices are set by the manufacturer. Biktarvy has been selected for Medicare price negotiation, with any new Medicare price taking effect in 2028.

Is generic HIV medicine the same?

A licensed generic contains the same active ingredients in the same doses as the brand. Licensed manufacturers make them to WHO, European or US quality standards. Whether you can legally import one depends on your country’s rules.

Is there a generic Biktarvy in the US?

No. DailyMed lists no generic version as of October 2026. Generic versions are made in India under licence for low- and middle-income countries.
