What is Abacavir?

Abacavir (ABC) is a nucleoside reverse transcriptase inhibitor (NRTI) used in combination ART for HIV-1. It is often used in fixed-dose combinations such as Kivexa (abacavir/lamivudine) and Triumeq (abacavir/dolutegravir/lamivudine). Patients must be screened for the HLA-B*5701 allele before starting, as carriers are at high risk of a serious hypersensitivity reaction.

Mechanism

How Does It Work?

Intracellular activation

Abacavir is converted intracellularly to carbovir triphosphate (CBV-TP), the active form, via cellular enzymes.

Reverse transcriptase inhibition

CBV-TP competes with dGTP for incorporation into viral DNA by HIV reverse transcriptase, causing chain termination.

Viral suppression

Without functional reverse transcriptase, HIV cannot convert its RNA into DNA, preventing integration into the host genome.
