What is Sirolimus?
How Does It Work?
Sirolimus binds the intracellular protein FKBP12, forming a drug-protein complex that is the active immunosuppressive species.
The sirolimus-FKBP12 complex binds and inhibits mTORC1, a critical kinase that regulates cell growth, metabolism, and T-cell proliferation in response to IL-2.
Inhibiting mTOR prevents T-cell transition from G1 to S phase, suppressing clonal expansion and immune-mediated rejection without directly blocking calcineurin.
How to Use
- Loading dose 6 mg on day 1, then 2 mg/day; adjust by whole-blood trough levels
- Target trough: 4–12 ng/mL (year 1), 4–8 ng/mL (thereafter)

Take exactly as prescribed. Your prescriber's instructions replace the general guidance above.
