About this medicine

What is Alecensa 150 mg Capsules?

Alectinib is a targeted medicine in capsule form for non-small cell lung cancer whose cells carry a rearranged ALK gene ('ALK-positive'). It is used as a first treatment for advanced ALK-positive lung cancer, after crizotinib has stopped working, and — more recently — after surgery for early-stage ALK-positive disease to reduce the chance of the cancer returning.

ALK-positive lung cancer often spreads to the brain, and alectinib was designed to cross into the brain, where many earlier drugs could not reach. Your tumour must have been tested for the ALK rearrangement before it is prescribed.

Alectinib is taken twice a day with food, continuously. Muscle aches, constipation, swelling and raised liver enzymes are the side effects your oncologist watches for. It is a prescription-only medicine; a valid prescription must be provided at checkout.

How it works

How does Alectinib work?

In ALK-positive lung cancer a piece of the ALK gene has fused to another gene, producing a permanently switched-on ALK protein that tells the cell to keep growing. Alectinib enters the cell and blocks the signalling part of that fusion protein, so the growth message stops and the cancer cell dies. It is more selective than crizotinib and still works against several of the ALK mutations that make tumours resistant to crizotinib. Because it is not pumped back out of the brain by the blood–brain barrier's transporters, it reaches brain metastases in useful amounts.

ALK blockade

Alectinib occupies the ATP pocket of the ALK fusion kinase (and RET), stopping it from activating downstream STAT3, PI3K/AKT and MAPK signalling.

Resistance coverage

It remains active against several crizotinib-resistance mutations, including L1196M and G1269A.

Brain penetration

Alectinib is not a P-glycoprotein substrate, so it is not exported from the central nervous system and reaches brain metastases.

Dosage

How to take Alecensa 150 mg Capsules

Your oncologist will prescribe your exact dose. The right dose for you depends on your weight, kidney and liver function, and — for some medicines — your genetic test results.

Always take exactly as prescribed. Do not change your dose or stop taking this medicine without speaking to your doctor first, even if you feel well.

Missed a dose? Skip a missed dose and take the next one at the usual time. If you vomit after a dose, do not replace it. Never double up.

With food? Take with food — a meal roughly triples absorption and the approved dose assumes it. Swallow capsules whole with water; do not open or chew.

Important safety information

Safety & warnings

Please read this section before taking Alecensa 150 mg Capsules and discuss any concerns with your oncologist.

Common side effects

Side effects affect some people, not everyone. Most are manageable — let your treatment team know about anything that concerns you.

  • Constipation
  • Muscle aches and raised creatine kinase (CK)
  • Swelling of hands, feet, face or eyelids (oedema)
  • Tiredness (fatigue)
  • Anaemia
  • Raised liver enzymes and bilirubin
  • Weight gain
  • Rash, sun sensitivity
  • Slow heart rate (bradycardia), usually without symptoms
  • Vision changes (blurring, floaters)

Contact your doctor immediately if you notice:

  • Yellow skin or eyes, dark urine, pain under the right ribs Contact your oncologist immediately — possible liver injury; liver tests every two weeks for the first three months.
  • Unexplained muscle pain, tenderness or weakness — especially with dark urine Contact your team the same day — CK is checked every two weeks for the first month; severe muscle breakdown is rare.
  • New or worsening breathlessness, dry cough, fever Seek urgent review — lung inflammation (interstitial lung disease) is rare but serious.
  • Dizziness, fainting, very slow pulse Seek review — heart rate is monitored; blood-pressure and heart medicines may need adjusting.
  • Flank pain, blood in urine, passing much less urine Contact your team — kidney injury has been reported.
  • Severe blistering skin reaction Stop and contact your team immediately.
  • Sunburn after brief sun exposure Use SPF 50+ and cover up during treatment and for a week after; report severe reactions.

Important: tell your doctor about these medicines

Taking Alectinib with certain other medicines can be dangerous. Always tell every doctor, dentist, and pharmacist about all medicines you are taking.

    Additive bradycardia.

  • Live vaccines Major

Safety not established during active treatment.

FAQ

Frequently asked questions

Do I need an ALK test before taking Alecensa 150 mg Capsules?

Yes. Alectinib only helps lung cancers with an ALK gene rearrangement — about 4–5% of non-small cell lung cancers — and your oncologist will have confirmed it with a validated test on your tumour before prescribing.

[1] FDA prescribing information: Alecensa (alectinib) — DailyMed

Why must Alecensa 150 mg Capsules be taken with food?

Food increases absorption around three-fold, and the approved 600 mg dose assumes a meal. Taking it fasting would deliver much less drug than intended.

[1] FDA prescribing information: Alecensa (alectinib) — DailyMed

How is Alecensa 150 mg Capsules different from crizotinib?

Both block ALK. Alectinib is more selective, covers several of the mutations that make tumours resistant to crizotinib, and — importantly — reaches the brain, where ALK-positive lung cancer often spreads. In head-to-head studies it was used first-line ahead of crizotinib. Your oncologist chooses based on your history and what is available.

[3] EMA: Alecensa — European public assessment report

Why are my liver and muscle enzymes checked so often?

Alectinib commonly raises liver enzymes and creatine kinase (CK), usually without symptoms. Liver tests are done every two weeks for three months and CK every two weeks for the first month, so a dose pause can be made before anything serious develops. Report muscle pain or dark urine between tests.

Why do I sunburn so easily now?

Alectinib makes skin more sensitive to ultraviolet light. Use a broad-spectrum SPF 50+ sunscreen, cover up, and avoid midday sun during treatment and for at least a week after stopping.

[4] Macmillan Cancer Support: Alectinib (Alecensa)

Is Alecensa 150 mg Capsules chemotherapy?

No. It is a targeted therapy that blocks one faulty protein. Hair loss and severe sickness are not typical; constipation, muscle aches, swelling and tiredness are the usual effects.

[2] NCI Drug Information: Alectinib

Do I need a prescription to order Alecensa 150 mg Capsules?

Yes. Alectinib is prescription-only. Upload your oncologist's prescription at checkout; our pharmacist team reviews it before dispatch. Used as prescribed by a licensed medical professional.
